The following lists help generally characterize a new presentation.

OPQRST

  • O - Onset
  • P - Provoking and palliating factors
  • Q - Quality ("What does it feel like?")
  • R - Radiating (noting location of origin)
  • S - Severity (usually use a scale from 0-10)
  • T - Timing (constant, intermittent, duration, time of day)

OLD CARTS

  • O - Onset
  • L - Location
  • D - Duration
  • C - Characteristics ("What does it feel like?")
  • A - Aggravating or alleviating factors
  • R - Radiating
  • T - Timing
  • S - Severity (scale from 0-10)

SOCRATES

  • S - Site
  • O - Onset
  • C - Characteristics ("What does it feel like?")
  • R - Radiating
  • A - Alleviating
  • T - Timing
  • E - Exacerbating factors
  • S - Severity (scale from 0-10)
